We need to know how we are performing today in order to improve our performance in the future. Improvements may result in lowering operating overhead costs, increasing throughput, pushing out time between shutdowns, de-bottlenecking, minimizing waste, etc. We can therefore with relative ease apply a measurement system not only to measure the results of the facility integrity processes but also to measure internal performance of our integrity processes themselves. Ultimately, we are driving towards an answer to the following questions: How is the facility integrity department performing?
In response to this problem, an existing vent point on the pipe was modified to include the injection of water into the line. The idea was that the water injected into the pipe would dissolve the salts to address the fouling problems. It was discovered that, over time, the water injection into the vent impinged the pipe elbow in a corrosion/erosion degradation mechanism and ultimately ruptured the elbow. On investigation it was noted that the overhead gas pipe elbow wall thickness had reduced to such an extent that the wall could no longer withstand the internal pressure within it.
As we apply the barrier analysis model, it is apparent that weaknesses within each barrier tend to be present in some shape or form.
